Steps To Receiving Disability Care With Medicaid Coverage

Phone Consultation

Share your situation and care needs so we can understand how to best support you and verify your Medicaid coverage or HMO plan.

Authorization & Coordination

We or your case manager will send an authorization request to your HMO. This process may take up to three weeks, depending on the plan. Once authorization is received, we confirm your active Medicaid status through the NJ Medicaid Portal, obtain your Medicaid number, and complete your client profile.

In-Home Assessment

A nurse visits to evaluate safety, health needs, and daily routines, then builds your personalized Plan of Care.

Caregiver Match

We pair you with a compatible, qualified caregiver.

Start of Services

Your caregiver starts services once your plan is approved.

Steps To Receiving Disability Care With VA Coverage

Tell us about your care needs and VA benefits. We’ll help verify your eligibility or coordinate directly with your VA case manager for authorization.

Authorization & Coordination

Once approved, we receive the VA authorization and finalize the service plan aligned with your coverage level.
